Resolution:  
There are two places to enable require comments for hours when entering a timesheet.
 
Option #1:  To require comments on all timesheets for all employees on all projects:
  1. Go to Configuration > Time & Expense > Company Timesheet > Setup tab
  2. Check the box for Require comments when hours are entered.
  3. Select Save at the top of the window.  
Option #2:  Setup to require comments on certain projects.  This setting overrides the default options set in Configuration > Time & Expense > Company Timesheet:
  1. Go to Info Center > Projects > Time & Expense tab and select the lowest level of the project.
  2. Select Yes in the Required Comments for Hours drop-down field.
  3. Click Save.
